GENERAL QUESTIONS

How many residents live in a room?

The majority of the cottages have single bedrooms.

When can I visit?

We have no set visiting hours. We ask that you take the resident's
schedule into consideration. Please contact Social Services or
the Cottage Supervisor prior to visiting.

Can I take my loved one home for a visit?

Of course! Residents may leave campus for the day or for overnight
stays with family. We need advance notice when you will be taking
the resident off campus so that we can assure having enough medication
on hand and rearrange his/her programming schedule. Due to state
regulations, we may also need to limit the number of overnight stays
for residents receiving Medicaid.

Can I bring my loved one his/her own clothing?

Please do! The clothing will be marked with his/her initials and a cottage
color. If you so choose, you may also provide clothing on a continuing basis.

FINANCIAL INFORMATION

Some residents enter the Home on a private-pay status. This rate changes
periodically so you should check with our Social Services Department for
the most current cost per day.

The majority of our residents are on some form of public assistance, such
as Medicaid, Social Security, etc. Some residents come here to live and
are already on assistance. Others apply for assistance once they become
residents.

There are eligibility requirements for receiving financial assistance.
Our Social Services Department will be able to help you decide whether
you should pursue some type of financial assistance and facilitate contact
with the Ottawa County Department of Job and Family Services.

ADMISSIONS PROCESS

STEP 1 Contact the Social Services Department to indicate you have a need
at 419-836-7741.

Step 2 If a Social Worker feels that Luther Home of Mercy would be an
appropriate placement possibility, you will be asked to come for a tour and
receive an application.

Step 3  When the application is received by our Social Services 
Department, it is reviewed and placed on our waiting list.

STEP 4 When an appropriate vacancy occurs, you will be contacted to
confirm the need for placement. Social Services will make an appointment
for our admissions team to visit the prospective resident in his/her current
living situation.

STEP 5 After the visit, you will be informed as to whether we will pursue
a placement. You will be informed of all the necessary information needed
for the admission to take place.

STEP 6 The specific time for placement to occur will be mutually agreed
upon by our staff and your family.

Vacancies at Luther Home of Mercy are filled from the waiting list by
URGENCY OF NEED and potential habilitation needs, so it is important that
you keep the social worker informed of changes in your situation.

The complete admissions policy is available from the Social Services
Department upon request.

At Luther Home of Mercy our goal is to help each resident develop to his/her
fullest potential, to be as independent as possible. Residents are evaluated
annually by an interdisciplinary team. The team, along with the resident and
his/her family, evaluate how best to proceed with programming. Some of

Through programming, we work to develop the total person; striving
to help the emotional, spiritual and physical components all work together
as a unit. We want to help our residents be the best they can be...by
allowing risk...success...even failure, all components in the development process.
